The type of personal information we collect
We currently collect and process the following personal information:

· Date of birth
· Address
· Phone Number
· G.P Contact details
· Name, address, and contact details of third parties supplied by you, the client. An Assessment form comprising personal details, a brief history of any health conditions, medication, personal and family relationships, life, and risk.
How we obtain personal information and the reasons for it
All of the personal information we process is provided to us directly by you for the following reasons:
To assess emotional and mental health and suitability for counselling.
To ensure we have contact details in case of an emergency.
To maintain a history of your life.
To identify what you hope to gain from counselling.
We use the information that you provide to us in order to:
Offer a service within the boundaries of confidentiality and the law, adhering to our privacy policy.
Fulfill our responsibility to you and the general public.
Comply with the ethical guidelines set forth by the British Association of Counsellors and Psychotherapists.
Meet our insurance requirements.
We may share this information with:
A UK court if legally requested to do so.
Your G.P. or emergency services in cases of harm to self or others.
My supervisor in the event of my death.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent. You may withdraw your consent at any time by contacting Kevin Powell.
(b) We have a contractual obligation.
(c) We have a legal obligation.
How we store your personal information
Your information is securely stored, and I keep:
Telephone numbers, first name only and number coded, on a password-protected mobile phone. These are deleted two weeks after our work together has ended.
Text messages, first name only and number coded, on a password-protected mobile phone. These are deleted two weeks after our work together has ended.
Emails and associated information we have exchanged, kept on a password-protected mobile phone and computer. Our email provider is encrypted. These are deleted two weeks after our work together has ended.
Written documents, notes, assessments, and other forms sent to you, kept locked in a metal filing cabinet which I have sole access to. These are destroyed after five years in accordance with my insurance provider's requirements.

Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to request corrections to any inaccurate personal information. You also have the right to ask us to complete information you believe is incomplete.

Your right to restriction of processing - You have the right to request a restriction on the processing of your personal information in certain circumstances.
Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to request that we transfer your personal information to another organization or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
